Party Participation and Liability Waiver Agreement

This form must be completed and signed by all parents, legal guardians, or adult participants prior to entering or participating in any activities at Jump Bounce Go Kidz Zone LLC, including hosting or attending birthday parties. This waiver will be valid the entire calendar year and will cover all participation in and/or hosting of birthday parties and related activities at the facility. A new waiver is required to be filled out annually.

THIS IS A LEGAL DOCUMENT

This Agreement is entered into between the undersigned parent or legal guardian (“Parent”) and Jump Bounce Go Kidz Zone LLC (“the Facility”), regarding participation in and/or hosting of birthday parties and related activities at the facility.

Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge and understand the following:

1. Voluntary Participation: I am allowing my child(ren), and/or child(ren) in my care (including those not biologically mine), to voluntarily participate in activities at Jump Bounce Go Kidz Zone LLC (“the Facility”), including but not limited to:

   • Jungle gym/climbing structures

   • Ball pits

   • Rock climbing walls

   • Indoor play equipment

   • Party area activities

   • Use of restroom and common facilities

2. Physical Activity Risk: I understand that participation in these activities may involve inherent risks including, but not limited to, slipping, falling, collisions with equipment or other children, minor or serious physical injuries, or exposure to illness or other unforeseen hazards.

Responsibility for Non-Biological Children: I acknowledge that I may bring or be responsible for children who are not biologically mine. I affirm that I have obtained proper authorization from their parent/legal guardian to act in their stead and to sign this waiver on their behalf. I accept full responsibility for supervising and ensuring the safety of all such children in my care while on the premises.

Release of Liability and Indemnification

In consideration of being permitted to enter and use the facilities at Jump Bounce Go Kidz Zone LLC:

• I, on behalf of myself, my child(ren), and any guests in my care hereby release, waive, and discharge Jump Bounce Go Kidz Zone LLC, its owners, employees, managers, affiliates, contractors, and volunteers from any and all claims or liabilities for injuries including death, damages, or losses to any person or property which may arise from participation in the above-mentioned activities or while on the premises whether inside or outside.

I further agree to indemnify and hold harmless Jump Bounce Go Kidz Zone LLC from any and all claims brought by or on behalf of the child(ren) in my care or any third party, including claims based on the negligence of the facility, its staff, or participants.

Rules and Conduct

• I agree to supervise my child(ren) and any child(ren) I bring at all times unless other supervision is provided by the host or staff.

• I agree to follow all posted rules and staff instructions for the safety of all guests.

• I understand that rough play, unsafe behavior, or noncompliance with facility rules may result in removal from the premises without refund.

Emergency Care Authorization

In the event of an emergency, I authorize Jump Bounce Go Kidz Zone LLC to provide basic first aid and/or contact emergency medical services as needed for the child(ren) listed above. I accept responsibility for any resulting medical expenses.

I certify that I have read and fully understand this agreement, that I am 18 years of age or older, and that I am the legal parent/guardian or authorized caregiver for the child(ren) listed. I voluntarily agree to the terms above.
